Your Visit to Our Office

Welcome to Our Practice

If you are scheduled to see one of our providers for a new patient visit or follow-up visit, you will come to our office at 488 East 11th Ave, Suite 150A in Eugene (Get Driving Directions).  We are located on the ground floor of the Oregon Community Credit Union building. In order for us to serve you in the best possible manner, we request that you bring the following to your first office visit:
  • Any pertinent medical records
  • Your most current insurance information/insurance card
We also ask that you arrive early to allow time for parking and locating our office. We do our best to accommodate every patient to the best of our ability; however, if you are late for your appointment, we may have to reschedule you for another day. If you miss your appointment and fail to notify our office with 24 hours advance notice, you may be subject to a $50 cancellation fee.

Patient Forms

You may be asked to complete any or all of the forms below depending on the nature of your visit. Our office staff will identify which forms are needed. For your convenience, you can download these forms and fill them out in advance to bring with you to your appointment.
